Position Summary
Under administrative direction, the purpose of the position is to direct and administer the case management functions of a community paramedicine program within the Fire Rescue Division of the Walton County Sheriff’s Office. The Community Health Case Coordinator is responsible for managing and coordinating implementation, operational needs, scheduling of patients, regulatory compliance, and resource management issues for an assigned technical support operation. Provides integrated health services and extends access to health services in underserved and general populations. Performs technical work in the provision of basic medical care under the direction of a physician, by written protocol or direct communication.
Pay Range Details
LPN - $50,000
RN - $57,500
Paramedic - $55,000
Qualifications
- Applicants must have a minimum of five (5) years of service as a licensed practical nurse, registered nurse, or paramedic.
- Cardiopulmonary Resuscitation (CPR) for Healthcare Professionals
- Active Florida licensed practical nurse, registered nurse or paramedic license.
- Advanced Cardiovascular Life Support (ACLS) or obtain within 1 year.
- Pediatric Advanced Life Support (PALS) or obtain within 1 year.
- A valid driver license.
- Must have successfully completed a community paramedic, critical care specialty course or approved equivalent within 1 year of hire.
- Must have working knowledge of all rules, regulations and information that is necessary for successful job performance.
- Must have the ability to exercise sound judgment and prepare clear and concise reports.

- Regular and prompt job attendance
- Performs highly responsible duties serving as an advisor to the Division Chief of EMS regarding the community paramedic program.
- Assists the Division Chief of EMS in developing annual community paramedicine operational goals and objectives and recommending equipment purchases.
- Develop, analyze, and implement budgets, grants, policies, procedures, plans and programs to address health care gaps existing within Walton County.
- Develops and maintains an effective system of records and reports for the community paramedicine program and other clinical aspects. Provides annual appraisals on those program activities.
- Coordinates and manages all community health case management activities within the Fire Rescue Division to include but not limited to, state licensure, communication with the medical director, development and implementation of medical protocols, community paramedicine, medical quality assurance, implementation of new medical technologies, etc.
- Continually evaluates the community paramedicine program’s effectiveness in case management, as well as overall outcomes, keeping the Division Chief of EMS apprised of important developments or recommendations for improvement.
- Develop and maintain relationships with acute and long-term care providers.
- Performs routine community needs assessments and adjusts the case management delivery model of the community health program.
- Meets with both internal and external stakeholders on a routine basis for all programs managed to determine the effectiveness of our current service delivery model.
- Establishes and maintains the highest level of customer service by providing a positive working relationship with all stakeholders.
- Functions as a licensed practical nurse, registered nurse, or paramedic as directed by Medical Director approved Medical Protocols.
- Abides by all laws and regulations regarding storage and handling medications and generating reports.
- Participates in training and mentoring of new employees as needed relating to the community paramedicine program and telehealth services.
- Performs routine in-home patient visits as well as provides services for patients.
- Adheres to work schedule and attendance requirements.
- Performs other duties as necessary and required.
Required Skills
- Tasks involve the intermittent performance of extremely physically demanding work, typically involving some combination of reaching, bending, stooping, kneeling, crouching, running, climbing, and that may involve the lifting, carrying, pushing, and/or pulling of extremely heavy objects (150+ pounds), such as in the event of rescuing or transporting a victim/patient in the performance of rescue operations.
- Considerable knowledge of basic, ACLS, PALS, as well as basic and advanced medical procedures and techniques.
- Considerable knowledge of the principles, practices, and emerging trends of modern case management and nursing programs.
- Considerable knowledge of program development and administration techniques.
- Considerable knowledge of modern medical technology, computer operating systems, and software applications.
- Working knowledge of County geography and of the location of roads and streets within the County.
- Working knowledge of radio communication equipment.
- Skills in diagnosing a victim’s condition and applying appropriate emergency medical care.
- Ability to respond quickly and calmly to medical situations.
- Ability to safely drive agency apparatus under all conditions.
- Ability to understand and follow oral and written instructions.
- Ability to prepare and maintain accurate records and reports.
- Ability to lift substantial weight.
- Ability to deal tactfully, courteously, and firmly with the general public.
- Ability to effectively establish and maintain working relationships as necessitated by work assignments.
- Ability to establish and maintain effective working relationships with other employees and volunteers, supervisors, other Fire Chiefs, and the public.
- Ability to remain clear-headed and diplomatic in stressful situations.
- Ability to provide effective written and verbal communications to supervisors.
